Honor an Educator Donation Form 

Please send completed form with your check to the Foundation.

Just print and complete the form below and send your check payable to: Lawrence Township School Foundation, 5626 Lawton Loop E Drive, Indianapolis, IN 46216.  All donors will receive an acknowledgment with a receipt for their tax-deductible gift.

Forms and donations must be returned to the Foundation office in order for the honorees to receive their acknowledgments.
